Complete this form to apply for benefits or reopen an unemployment claim. The DE 1101I can only be printed and completed in English or Spanish, and faxed or mailed to the EDD.

Choose the application that best matches your job situation. If a disaster has affected you, fill out the disaster section of the unemployment application.

Authorization for Release of Unemployment Insurance Records for Retired Annuitant
This form is for former State of California Civil Service employees who are now Retired Annuitants. The Retired Annuitant completes the form to allow us to release unemployment records. The hiring state agency checks if the person received benefits in the last 12 months.

Wages Notice Request – Holiday/Vacation Pay (Work Sharing and Non-Work Sharing Employer)
Employers fill out this form if, on the last day of work, some or all employees will have a set date to return to work and will get holiday or vacation pay. Return to the department by mail or fax to the contact information found at the bottom of the form.

Employers fill out this form if they are paying some type of bonus pay to affected employees. The form can be completed online, printed, and mailed or faxed.

Employers fill out this form only if we sent a previous Wages Notice. The form is used to correct information on a current notice or provide details about a recent layoff. The form can be completed online, printed, and mailed or faxed to EDD.

Notice of Unemployment Insurance Claim Filed (Employer)
Sent to a person’s last employer when a new claim is filed, or an existing claim is reopened. The employer may receive either a DE 1101CZ or a DE 1101ER. The form has information that is specific to the claimant. The last employer fills out parts of the form and sends it to the EDD. (The forms on this webpage are just samples. Don’t use them to send information.) Employers can also send information in a letter. Include the claimant’s Social Security Number (SSN) and use the return address on the DE 1101CZ/ER, or DE 1545 for mailing. The last employer can electronically receive and respond to the EDD’s Notice of Unemployment Insurance Claim Filed (DE 1101CZ) using SIDES. See SIDES FAQs.
